Multiple contributing factors — including OceanGate’s toxic workplace environment and profit-over-safety ethos — led to the entirely "preventable" implosion of a submersible that killed five people off Newfoundland two years ago, Coast Guard investigators said Tuesday.

Five people died when OceanGate's Titan submersible catastrophically imploded during a deep-sea voyage to the Titanic wreckage in 2023.

The disaster was preventable and was ultimately caused by OceanGate Expeditions cutting corners, not meeting safety and engineering standards, according to the report.
